VideoMD is a multimedia platform for doctors to help educate their patients better. We use PEEAAS, patient education and engagement as a service to achieve this goal. We developed the program to help hospitals lowers their 30 day readmission rates using patient education and engagement. Patients are registered on the day of discharge and begin a month long, web based, educational program. We track patient generated data, which are reported to the hospitals and the outpatient doctors. Our platform facilitates collaboration between doctors and hospitals during the difficult transition of care from inpatient treatment to chronic outpatient management. Our goal is to educate patients, improve care and lower the 30 day readmission rates for hospitals. We also help the outpatient physicians increase revenue, by helping them provide the care, and bill for the new, more lucrative, Medicare Transitional Care Codes. We are getting ready to start our first hospital based pilot shortly.
